In Italy, TransForm is promoted by our Foundation in collaboration with the Piedmont Region, La Bottega del Possibile Association and the Emanuela Zancan Foundation.<\/p>\n<p class=\"p1\">The initiative aims to explore the potential and promote the adoption of an <b>approach to health and care that is centred on communities<\/b>, recognising and supporting their role and that of every citizen in the practice of care and in promoting health and well-being, in the context of multidisciplinary, interprofessional and intersectoral collaborations<b>.<\/b> Great attention is paid to the issue of health inequalities and the role of the social, economic and environmental determinants of the health of individuals and communities.<\/p>\n<p class=\"p3\">TransForm promotes a holistic concept of <b>care that is centred on the individual and their needs and wishes, the potential and resources they are able to express<\/b>. The heart of the approach is the combination of medical care with social care, creating new forms of collaboration between different contexts, where the role of the community is fundamental. This involves a change of paradigm: from traditional models, based on identifying a problem or illness (problem-based) and establishing the consequent treatment plans (disease-oriented), to an approach that aims to promote the health of an entire community and to involve the individual in defining their life goals and improving their conditions (goal-oriented).<\/p>\n<p class=\"p4\">The TransForm project involves local seminars and international conferences attended by professionals and operators, researchers, decision makers and people with policy making and advocacy tasks, on issues related to the subject of international conferences and specific practices at local level. The conferences held so far have shown concrete examples of the application of the ICC approach by presenting case studies, including visits to places hosting innovative local projects.<\/p>\n<p class=\"p4\">These conferences have revealed a widespread awareness \u2013 among experts, operators and policy-makers \u2013 of the <b>need for a change of paradigm<\/b> to focus on communities and individuals, not only by <b>making interventions relevant to the specific local needs<\/b> but also <b>by involving local and actors and citizens <\/b>in the processes themselves.
